Consent Form

Below are the questions you will find on our Consent Form. On the day of your appointment you will be asked to confirm all of the below statements to ensure we can complete your tattoo safely. If you have any questions about these requirements before your appointment please get in touch

I am not under eighteen years old and can provide a nationally recognised form of identification if asked.

I am not pregnant or nursing (breast feeding).

I am not under the influence of alcohol or drugs.

I have not received any Flu or COVID vaccinations within the last 4 weeks.

I do not have any condition(s) that might affect the healing of this tattoo.

I do not have any blood transferrable diseases or infections.

I do not have any skin conditions or medical conditions such as but not limited to; acne, scaring, keloid, eczema, psoriasis, freckles, moles, sunburn, infection, rash that may interfere with my tattoo.

I do not have any physical, mental or medical impairment or disability which might affect my well-being or my ability to remain still during tattooing, as a direct or indirect result of my decision to being tattooed or having a tattoo.

I understand that a tattoo is a permanent change to my appearance.

I understand that variations in tattoo colour, brightness and quality may exist between the design and final tattoo applied to my body due to items such as, but not limited to; skin colour, skin health, skin quality, age, my ability to remain still during tattooing and my aftercare adherence.

I understand that any skin altering procedures, including but not limited to, laser hair removal, plastic surgery, rapid weight loss or gain may result in changes to my tattoo.

I understand it is not reasonably possible for Made For You Tattoo or its employees, to determine whether I might have an allergic reaction to the pigment or processes used for my tattoo and therefore accept the risk that such a reaction is a possibility.

I understand that I will receive aftercare instructions post tattooing and I agree to follow them strictly whilst my tattoo is healing, which will take approximately 14 days.

I understand that infection is always possible as a result of obtaining a tattoo and infection particularly occurs if I do not take proper care of my tattoo as outlined in the aftercare instructions provided.

I understand that any ‘touch-up’ work necessary due to my negligence whilst healing will be done at my own expense.

I have been given the full opportunity to ask any and all questions I might have about the procedure of obtaining a new tattoo and all my questions have been answered to my full satisfaction.

I have truthfully represented to my Tattooer that obtaining my tattoo is by my choice alone.

I consent to the application of the tattoo and to any actions or conduct of the representatives and employees of Made For You Tattoo, deemed by them, reasonably necessary to perform the tattoo procedure.
